Background: Universal health coverage is one of the Sustainable Development Goal targets known to improve population health and reduce financial burden. There is little qualitative data on access to and quality of primary healthcare in East and West Africa. The aim of this study was to describe the viewpoints of healthcare users, healthcare providers and other stakeholders on health-seeking behaviour, access to and quality of healthcare in seven communities in East and West Africa. Methods: A qualitative study was conducted in four communities in Nigeria and one community each in Kenya, Uganda and Tanzania in 2018. Purposive sampling was used to recruit: 155 respondents (mostly healthcare users) for 24 focus group discussions, 25 healthcare users, healthcare providers and stakeholders for in-depth interviews and 11 healthcare providers and stakeholders for key informant interviews. The conceptual framework in this study combined elements of the Health Belief Model, Health Care Utilisation Model, four ‘As’ of access to care, and pathway model to better understand the a priori themes on access to and quality of primary healthcare as well as health-seeking behaviours of the study respondents..............
Background: With an increasing number of patients who visit emergency outpatient units, a shortage of physicians and nurses in emergency units has become an issue in Japan. Triage nurses who interview patients and their families before medical examinations feel stressed to determine the triage level in a limited time, necessitating the measures to alleviate stressors. Objective: To develop a triage nurse job stressor scale (TNJSS) for Japanese triage nurses in emergency outpatient units and to verify the reliability and validity of this scale. Methods: Anonymous, self-administered questionnaires were sent to nursing directors of 180 emergency and critical care centers randomly selected from 251 centers throughout Japan, requesting to distribute the questionnaire to nurses. Results: Based on the responses obtained from 363 nurses, the construct validity, internal consistency, and criterion-related validity were verified. A factor analysis of 44 items yielded five factors: “Lack of triage ability”, “Busy triage work”, “Patients without understanding of explanations”, “Complaints from patients waiting..................
During the height of the Coronavirus Disease (COVID-19) Pandemic, nurses all over the world, including members of the National Association of Nigerian Nurses in North America (NANNNA) experienced a heightened and unprecedented level of stress, anxiety, fear, uncertainty, concern for their safety and that of their loved ones. This prompted NANNNA leadership to initiate and conduct support programs for nurses especially those on the frontline of care. This project evaluates the effectiveness of the support programs conducted by NANNNA during the early surge of COVID-19. The Centers for Disease Control’s (CDC’s) Framework for Evaluation in Public Health guided this evaluation. Focus group discussion and online survey was utilized to gather data. While 19% of respondents reported being diagnosed with COVID-19, the majority (81%) were not.................
Background: Humanwide was precision health embedded in primary care aiming to leverage high-tech and hightouch medicine to promote wellness, predict and prevent illness, and tailor treatment to individual medical and psychosocial needs. Methods: We conducted a study assessing implementation outcomes to inform spread and scale, using mixed methods of semi-structured interviews with diverse stakeholders and chart reviews. Humanwide included: 1) health coaching; 2) four digital health tools for blood-pressure, weight, glucose, and activity; 3) pharmacogenomic testing; and 4) genetic screening/testing. We examined implementation science constructs: reach/penetration, acceptability, feasibility, and sustainability. Chart reviews captured preliminary clinical outcomes. Results: Fifty of 69 patients (72%) invited by primary care providers participated in the Humanwide pilot. We performed chart reviews for the 50 participating patients. Participants were diverse overall (50% non-white, 66% female). Over half of the participants were obese and 58% had one or more major cardiovascular risk factor: dyslipidemia, hypertension, diabetes...................
Background: Primary care providers are pillars of China’s medical and health sectors. However, due to the gap between career expectations and reality, they enter a career plateau phase through excessive pressure. This study aims to examine the prevalence and associated factors of the career plateau of primary care providers in Heilongjiang Province, China, and proposes strategies to improve and promote their career advancement. Methods: Based on city-level GDP growth in the province, a questionnaire survey was conducted among 1500 primary care providers (effective response rate = 85.8%). Pearson’s chi-square analysis and binary logistic regression were used to analyse the factors associated with their career plateau.................
Anxiety among nurses attending patients at the forefront of the global coronavirus disease- 19 (COVID-19) pandemic is on the rise. Accordingly, examining the psychological response of nurses who have attended COVID-19 patients is necessary. This study aimed to categorize and characterize the psychological responses of nurses who have experienced COVID-19 patient care based on the Q methodology. The Q-sample extracted 34 statements, and the P-sample marked 22 nurses who had operated a screening clinic and experienced patient care in the local base hospitals treating COVID-19 using convenience sampling. The findings suggest that nurses who have experienced COVID-19 patient care fall into three categories based on the following psychological responses: (1) fear of social stigma, (2) anxiety about the risk of infection, and (3) burden of infection prevention and control nursing. Therefore, it is expected that the results of this study may serve as the basis for emotional support programs that are capable of understanding and mediating the psychological state of nurses who care for patients with COVID-19....
